My daughter is 4 months old and we still swaddle her?

My daughter is 4 months old and we still swaddle her. Lately she's been breaking her arms out of the top no matter how well I get it closed (its on of those swaddleme ones with the velcro). When her arms come out she wakes up, probably mostly because she knocks her pacifier out. Any suggestions on how to wean her from her swaddle and/or pacifier since theyre both the problem right now?
